Try to schedule medical appointments outside of work hours. If it's not possible, schedule the appointment at the beginning or end of the day. If your employer doesn't cover the lost time, SFM will cover up to two-thirds of your wage loss. You would need to send SFM your pay stub for this pay period.
You have received a sheet "Getting prescriptions filled." This lists the ExpressScripts website and toll-free number to find pharmacies near you that you should use. You also have received your personalized Prescription services identification card. Show your card to the pharmacist so he doesn't charge you. If you haven't received your identification card in the mail, please call your SFM claims representative.
You should not receive any medical bills if SFM has accepted liability for your claim. If you do receive a bill, turn it over to your supervisor at work and he or she can send it to SFM, or you can send it to SFM directly.
It's against the law to fire an employee because he or she was injured at work. But, like any other employee, you could lose your job if you fail to show up or your performance is poor.
Your employer should have identified some transitional jobs you can perform within your medical restrictions until you are able to return to your original job. If you have ongoing problems, a medical case manager may be assigned to work with you, your employer and physician.
If you are unable to work with your pre-injury employer or if your recovery is prolonged, a qualified rehabilitation consultant (QRC) will be assigned to work with you. The QRC will help you get back to work.
