Tips to help you through this
- Tell your supervisor all important details relating to your injury. The time, date, what you were doing, who was there--this is all important information for processing your claim for workers' compensation benefits.
- Make sure your SFM claims representative has your current address and phone number.
- Stay in touch with the person in your organization who handles workers' compensation claims. Keep him or her updated on doctor visits, any changes in your condition, and your plans to return to work.
- Discuss any changes in work or medical status with your SFM claims representative. Don't be afraid to call him or her if you have questions about your claim.
- Get updated work restrictions from your physician at each medical appointment and give them to your employer.
- Put your Social Security number, date of injury and claim number on all papers and forms sent to SFM.
- Use your personalized Prescription services identification card for any work-injury related prescriptions.
