To make sure your injury is handled as best as possible and your employer, doctor and SFM are in the loop every step of the way, follow these helpful tips.
- Get updated work restrictions from your physician at each medical appointment and give them to your employer.
- Stay in touch with the person in your organization who handles workers' compensation claims. Keep him or her updated with doctor visits, any changes in your condition, and your plans to return to work.
- Call your SFM claims representative at (952) 838-4200 or (800) 937-1181 if you have questions, if your address or phone number changes, or if your medical or work status changes. Be sure to put your date of birth, date of injury and claim number on all papers and forms sent to SFM.
