Standard Online Reporting Form

Self-insured employers not registered for CompOnline can use SFM's standard First Report of Injury form to report online.

It's easy. No login or password is required—all you need is your SFM policy number and the date of injury to get started.

Registered users can complete the First Report of Injury online through CompOnline Risk Management System.

Complete the paper First Report and fax it to (952) 838-2000 or (800) 944-1169. Make sure to complete the appropriate form for your state. Keep your fax confirmation sheet as evidence that you reported on time.
