New address for SFM medical bills — March 18, 2015
Starting April 13, medical providers should send all SFM work-injury-related medical bills and records directly to SFM. SFM will begin processing workers' compensation medical bills internally in April and will no longer outsource this service.
Claimant ordered to pay nearly $100,000 in fraud case — March 5, 2015
An SFM claimant who submitted forged timecards to receive temporary partial disability payments was ordered to pay SFM $96,579 in restitution to cover the fraudulently received benefits, as well as legal and surveillance expenses at his criminal sentencing hearing on Monday, February 23.
