Your employee is injured, now what? Report the injury to SFM as soon as possible—ideally, within 24 hours.

The fastest and most convenient way for TPA customers to report injuries is online, whether through the Standard Online Reporting Form, or as a registered user of CompOnline®.

Self-insured employers not registered for CompOnline can use SFM's standard First Report of Injury form to report online.

It's easy. No login or password is required—all you need is your SFM policy number and the date of injury to get started.

Registered users can complete the First Report of Injury online through CompOnline Risk Management System.
