Submitting your applications to SFM
SFM makes it easy to submit your applications. Once we receive the information about your client’s account, we’ll get you a quote as soon as possible.
Options for submitting an application
The fastest and most efficient way to submit applications is online through our SFM Agency Manager (SAM) portal.
You can also email it to applications@sfmic.com.
Required application materials
- Completed ACORD application (unless submitting through SAM)
- Currently valued three- to five-year loss runs
- Experience mod worksheet
- Additional narrative information is also helpful
Tips for quick application response
- Be sure the FEIN is included and accurate
- Give the insured name — don’t confuse it with the DBA name
- Include a description of operations and a website
- Provide the current and target premium
- Don’t leave the loss information blank — if none, enter “None”
