How to report workers’ compensation fraud

Fraud is a crime that raises costs for everyone in the workers’ compensation system, and SFM pursues offenders.

If someone provides misleading information or withholds facts to receive workers’ compensation benefits or to obtain a workers’ compensation policy under false pretenses, they’re committing fraud. SFM works with the National Insurance Crime Bureau and appropriate authorities to fight insurance fraud.

If you suspect fraud, please complete the form below, and it will be sent to our Special Investigations Unit for follow up. In most cases, SFM will make every effort to maintain the confidentiality of your information.
