Pharmacy First Fill Form

An overview of SFM's first fill prescription program for policyholders with insured locations in Iowa, Minnesota, Nebraska, South Dakota and Wisconsin. This program allows injured employees to receive a 30-day supply of work-injury related medication at the time of injury with no out-of-pocket expense.
