The fastest and most efficient way to submit applications is online through our SFM Agency Manger (SAM) portal. You can also email us at applications@sfmic.com.

Tips for quick responses

  • Make sure the FEIN is included and accurate
  • Provide employer legal name (not the DBA name)
  • Include employer’s website and description of operations
  • Provide the current and target premium
  • Include loss information; enter “None” if it doesn’t apply
