Report a TPA injury
Report a work injury as soon as possible using one of the methods below, even if you don’t know all the details at the time of the report. Any missing information can be addressed later with your SFM Risk Solutions claims representative. You’ll receive contact within 24 hours of reporting a claim. Our staff promptly handle, thoroughly investigate and fairly resolve your claims.
In an emergency, call 911 or get the employee emergency care before reporting the injury to SFM.
Report through CompOnline Registration required
If you’ve registered for CompOnline®, log in and save time completing the first report of injury there. Reporting through CompOnline is the most convenient way to begin the claims process.
Report online
Fill out the first report of injury form online. All you need is your SFM Risk Solutions policy number and the date of injury to get started.
Report by fax
- Complete your state’s paper First Report of Injury form:
