Report an injury as a Wisconsin assigned risk employer
As a Wisconsin Worker’s Compensation Insurance Pool employer, use one of the reporting methods below as soon as you learn that your employee has been injured. Any missing information can be addressed later with your SFM representative.
In an emergency, call 911 or get the employee emergency care before reporting the injury to SFM.

Fill out the first report of injury form online. All you need is your SFM policy number and the date of injury to get started.
