We insure employers in Minnesota, Wisconsin, Iowa, Nebraska, South Dakota, Kansas, Indiana and Tennessee. We also cover out-of-state operations these employers have in 26 other states including: Alabama, Alaska, Arizona, Colorado, Connecticut, Florida, Georgia, Idaho, Illinois, Kentucky, Louisiana, Maryland, Michigan, Mississippi, Missouri, Montana, Nevada, New Mexico, North Carolina, Oklahoma, Pennsylvania, South Carolina, Texas, Utah, Vermont and Virginia. SFM policy documents are made available to your agency electronically in either SFM Agency Manager (SAM) or through IVANs Download. You, as the agency, are responsible for delivering a copy of the policy documents to the insured. SFM does not deliver policy documents to policyholders.
In Minnesota, Wisconsin, Iowa, Nebraska, South Dakota, Kansas, Indiana and Tennessee. SFM’s payment plans are:
Premium greater than $5,000: 25% down and nine monthly payments
Premium from $1,500 to $4,999: 25% down and three quarterly payments
Premium from $500 to $1,499: 50% down and one semi-annual payment
Premium less than $500: annual payment
For direct billing, fees of up to $5 are added for each installment except for annual plans
Please note that higher premium levels also qualify for payment plans that lower premium levels are qualified for.
If you wish, yes. Otherwise, SFM will send a bill on a direct bill account.
Yes, employers billed directly by SFM have the option to pay premiums online.

Yes, SFM’s online wage reporting option may be available for all types of policyholders with at least $1,000 in premium. Policyholders can review previous wage reports and invoices and can make online payments. An initial collateral deposit may be required and adjusted at renewal.
If a collateral deposit is required then the following guidelines will be followed with some possibility for adjustment, depending upon policyholder specific circumstances: monthly 15%; quarterly 25%; semi-annual 50%.
